Cremation Forms

Form A is to be completed IN FULL by the Funeral Director, with the full address of the deceased.

Form B is to be completed IN FULL by a spouse, blood relative or legal executor. It cannot be completed by a partner, friend or carer, unless with written proof of the permission to act in that regard.

Question 3 on Form B should be answered ‘Yes’ to indicate that all relevant next of kin are aware of and are in agreement with the cremation taking place.

For cremations coming from R.o.I., Form C (Medical Forms) or Form D (Coroner’s Form) should accompany Forms A & B.

For cremations coming from N.I. or abroad, Form A & B need to be completed, along with the Verification of Infectious Disease Status Form and the Out of Country Certificate.
